Medical Social Worker Job Description

Apollo is seeking a Medical Social Worker to join our Home Healthcare Team. As a Medical Social Worker, you will provide quality home health visits to our patients in the comfort of their own homes. You will work collaboratively with a team of dedicated healthcare professionals to deliver exceptional care to our members.

Key Responsibilities:
  • Assess the psychosocial status of patients related to their illness and environment, and communicate findings to the registered nurse.
  • Carry out social evaluations and plan interventions based on evaluation findings.
  • Maintain clinical records on all patients referred to social work.
  • Provide information and referral services for Apollo patients and families/caregivers regarding practical and environmental needs.
  • Participate in the development of the total plan of care and case conferences as required.
  • Assist physician and other team members in understanding significant social and emotional factors related to health problems.
  • Participate in discharge planning.
  • Participate in inservice education programs.
Requirements:
  • Graduate of a bachelor's program in social work accredited by the Council on Social Work Education.
  • Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.
  • Minimum of one (1) year experience in healthcare.
  • Demonstrates good verbal and written communication, and organizational skills.
  • Possesses and maintains current CPR certification.
  • Ability to travel to different patients' homes.
  • Discretion in dealing with sensitive and confidential information.
