Physical Therapist for Home Health Visits

Apollo is seeking a skilled Physical Therapist to join our Home Healthcare Team. As a Physical Therapist, you will provide quality home health visits to our patients, working in the comfort of their own homes. You will be part of a team of dedicated healthcare professionals who are committed to delivering exceptional care to our members.
Key Responsibilities- Provide physical therapy services to patients according to a written physician's plan of care.
- Evaluate and assess therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
- Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.
- Report to the physician on patient's reaction to treatment or changes in condition, and initiate physical therapy programs as needed.
- Train patient in the use of prosthetic devices, identify patient and family/caregiver needs for other home health services, and refer as necessary.
- Prepare and submit clinical and progress summaries based on the attainment of goals, and participate in discharge planning for patients.
- Possess a degree from a Bachelor's or Master's program in physical therapy approved by an accredited organization.
- Licensed to practice as a physical therapist within the state, if applicable.
- Two years of appropriate experience as a physical therapist.
- Demonstrate good verbal and written communication and organizational skills, and possess current CPR certification.
- Ability to travel to different patients' homes, and discretion in dealing with sensitive and confidential information.
