Medical Social Worker for Home Health Visits

Apollo is seeking a compassionate and dedicated Medical Social Worker to join our Home Healthcare Team. As a Medical Social Worker, you will be responsible for providing quality home health visits to our patients in the comfort of their own homes.

You will be part of a team of healthcare professionals who are committed to delivering exceptional care to our members. The Medical Social Worker will be responsible for implementing standards of care for medical social work services.

Key Responsibilities:
  1. Assess the psychosocial status of patients related to their illness and environment, and communicate findings to the registered nurse.
  2. Carry out social evaluations and plan interventions based on evaluation findings.
  3. Maintain clinical records on all patients referred to social work.
  4. Provide information and referral services for Apollo patients and families/caregivers regarding practical and environmental needs.
  5. Provide information to patients or families/caregivers and community agencies.
  6. Serve as a liaison between patients or families/caregivers and community agencies.
  7. Maintain collaborative relationships with Apollo personnel to support patient care.
  8. Maintain and develop contracts with public and private agencies as resources for patient and Apollo personnel.
  9. Participate in the development of the total plan of care and case conferences as required.
  10. Assist physician and other team members in understanding significant social and emotional factors related to health problems.
  11. Participate in discharge planning.
  12. Participate in inservice education programs.
  13. Other duties as delegated by the Clinical Supervisor.
Requirements:
  • Graduate of a bachelor's program in social work accredited by the Council on Social Work Education. May have a Master's or doctoral degree in social work.
  • Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.
  • Minimum of one (1) year experience in healthcare.
  • Demonstrates good verbal and written communication, and organizational skills.
  • Possesses and maintains current CPR certification.
  • Ability to travel to different patients' homes.
  • Discretion in dealing with sensitive and confidential information.
  • Prior experience working in a home health setting is preferred, but not required.

Compensation:

Apollo currently pays $95-$125/visit and $30/hour for in-services, trainings, and orientations.
