Job Title: Coordinator of Care RN

At MJHS, we are committed to providing exceptional care to our patients and their families. We are seeking a skilled and compassionate Coordinator of Care RN to join our team.

About the Role:

The Coordinator of Care RN will be responsible for providing advanced clinical home care services to patients recovering from illness, injury, or surgery. This includes patient assessment, medication management, and patient/family education. The ideal candidate will have a strong background in acute medical-surgical nursing and experience working in a home care setting.

Responsibilities:
  • Provide a full range of advanced clinical home care services, including patient assessment, medication management, and patient/family education
  • Assume the ongoing, primary responsibility to coordinate, implement, and continually evaluate the home care needs of patients
  • Review patient insurance information and coordinate activities involved in each individual care plan
  • Share pertinent findings with family and professional caregivers
Qualifications:
  • Current NYS RN (Registered Nurse) license and registration required
  • Bachelor's degree in nursing preferred; Associate's degree required
  • Minimum of one year acute medical-surgical nursing experience required
  • Certified Home Health Agency (CHHA) experience preferred
  • Bilingual English and Spanish or Mandarin or Cantonese preferred
