As a member of our team, you will be responsible for coordinating and implementing individualized care plans for patients with complex medical needs. You will work closely with our interdisciplinary team to ensure that patients receive comprehensive care that addresses their physical, emotional, and social needs.

We are seeking a highly skilled and compassionate RN to join our team as a Care Coordinator RN. If you are passionate about delivering exceptional care and making a difference in the lives of our patients, we encourage you to apply.

Responsibilities:

  • Coordinate and implement individualized care plans for patients with complex medical needs
  • Work closely with the interdisciplinary team to ensure comprehensive care
  • Provide education and support to patients and their families
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Document patient progress and outcomes

Requirements:

  • Current NYS RN license and registration
  • Bachelor's degree in nursing preferred; Associate's degree required
  • Minimum of one year acute medical-surgical nursing experience required
  • Certified Home Health Agency (CHHA) experience preferred
  • Bilingual English and Spanish or Mandarin or Cantonese preferred
