We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Clinical Coordinator RN. As a Clinical Coordinator RN, you will play a vital role in ensuring the highest level of care for our patients with end-stage renal disease (ESRD) or chronic kidney disease.
About the RoleAs a Clinical Coordinator RN, you will be responsible for coordinating patient care and services, including trend lab values, tracking metrics on vital signs, weight, and outcome management. You will work closely with our interdisciplinary team to ensure the safety, comfort, and well-being of our patients.
Responsibilities- Coordinate ESRD patient care and services
- Ensure the highest level of care for patients
- Work closely with the clinical team to achieve quality outcomes
- Participate in quality improvement initiatives
- Current Registered Nurse (RN) license in the state of practice
- At least 18 months of registered nursing experience
- Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred
