As a Registered Nurse (RN) on our Senior Community Care team, you'll play a vital role in helping patients navigate the healthcare system and connect them to key support services. Your expertise will ensure accurate and complete documentation, effective communication with the interdisciplinary care team, and timely coordination of care.

Key Responsibilities:
  • Ensure accurate and complete documentation of patient information
  • Communicate and collaborate with the interdisciplinary care team to coordinate patient care
  • Assist the provider team with various care coordination activities, including benefit determination and therapy coordination
  • Participate in facility-based meetings and schedule family conferences, team meetings, and team case presentations
  • Coordinate training, tracking, and compliance with quality measures
  • Assist in creating and conducting in-services for selected audiences
  • Assist providers in ensuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation
Requirements:
  • Associate degree or higher in Nursing (RN)
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date
  • Certified in Basic Life Support
  • 3+ years of clinical experience in practice
  • Proven ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment
  • Access to reliable transportation
Preferred Qualifications:
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of geriatrics and chronic illness
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records
  • Proven ability to work with diverse care teams in a variety of settings
  • Proven effective communication skills and time management

New York Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors, including local labor markets, education, work experience, certifications, and more. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).
