Patient Care Coordinator

The Patient Care Coordinator plays a vital role in our team at Compassus, scheduling patient services and coordinating with staff to ensure seamless care delivery. This position requires a strong understanding of our Compassus values, including Compassion, Integrity, Excellence, Teamwork, and Innovation, as well as the ability to promote our philosophy and uphold the Code of Ethical Conduct.
Key Responsibilities
- Schedule patient services in a timely and accurate manner, documenting all interactions and updates.
- Collaborate with management and the patient interdisciplinary team to coordinate care and ensure effective communication.
- Provide exceptional communication to patients, staff members, healthcare professionals, and referral sources, responding to agency and patient needs in a professional and creative manner.
- Participate in on-call responsibilities as assigned, demonstrating flexibility and adaptability in a fast-paced environment.
- Perform other duties as assigned, contributing to the overall success of our team and organization.
- High school diploma or equivalent required; experience with patient scheduling and customer service preferred.
- Strong mathematical skills, including the ability to add, subtract, multiply, and divide in all units of measure, as well as compute rate, ratio, and percentage.
- Excellent language skills, with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, and governmental regulations.
- Strong written and verbal communication skills, with the ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
The demands of this role require regular standing, walking, and manual dexterity, as well as the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
