We are seeking a highly skilled and detail-oriented Care Coordinator to join our team at Gastromed, LLC.
Job Summary:
The Care Coordinator will be responsible for providing exceptional patient care and ensuring a smooth check-out process. This role requires strong communication and organizational skills, as well as the ability to work independently in a fast-paced environment.
Key Responsibilities:
- Assist patients upon check-out by reviewing physician orders and providing necessary instructions.
- Schedule patients for procedures and inform them of any required clearances.
- Coordinate diagnostic tests and ensure that all necessary documents are scanned to the patient's chart.
- Responsible for daily close-out reports and ensuring that all patients have been checked out or marked as no-shows.
- Perform other duties as assigned, including scheduling appointments and sending text messages to patients.
Requirements:
- High School Diploma or equivalent required.
- Bi-lingual English/Spanish preferred.
- 2-3 years of related experience in a medical setting preferred.
- Strong people skills and attention to detail.
- Ability to work independently and multitask effectively.
