Job Summary:

The Care Coordinator will be responsible for providing exceptional patient care and ensuring a smooth check-out process. This role requires strong communication and organizational skills, as well as the ability to work independently in a fast-paced environment.

Key Responsibilities:

  • Assist patients upon check-out by reviewing physician orders and providing necessary instructions.
  • Schedule patients for procedures and inform them of any required clearances.
  • Coordinate diagnostic tests and ensure that all necessary documents are scanned to the patient's chart.
  • Responsible for daily close-out reports and ensuring that all patients have been checked out or marked as no-shows.
  • Perform other duties as assigned, including scheduling appointments and sending text messages to patients.

Requirements:

  • High School Diploma or equivalent required.
  • Bi-lingual English/Spanish preferred.
  • 2-3 years of related experience in a medical setting preferred.
  • Strong people skills and attention to detail.
  • Ability to work independently and multitask effectively.
