Job Summary
The University of Miami/UHealth - UMHC/SCCC Site Disease Group is seeking a Per Diem Nurse Navigator to join our team. As a Nurse Navigator, you will be the initial point of contact for patients entering the health system, ensuring timely scheduling of appointments and coordination of care throughout the continuum. You will serve as a liaison between patients, families, caregivers, and the multidisciplinary care team, demonstrating commitment to quality patient care and implementing creative ways to meet diverse patient needs.
Key Responsibilities
- Triage new patient appointments to the appropriate provider(s) and schedule initial appointments in a timely manner.
- Review outside medical records for appropriate scheduling and assess barriers to care, referring patients to support services as needed.
- Education patients and families about disease processes, treatment options, and potential side effects, and assist with treatment decision-making.
- Collaborate with a multidisciplinary team to provide patients with supportive care services and ensure timely follow-up on treatment and supportive care recommendations.
- Facilitate shared decision-making with patients, caregivers, families, and the care team, and support patients throughout the care continuum.
- Participate in coordination of educational activities, including identifying and documenting individual patient barriers to learning, educating patients and families about disease processes and treatment options, and assisting with treatment decision-making.
- Administer psychosocial screening at patient intake and repeat screening as needed, making referrals to social workers, financial counselors, or support services as needed.
- Actively solicit and record feedback from referring physicians and their staff, developing recommendations for improving clinical operations to better meet their needs.
- Meet with physicians who have the potential to become referral sources for the cancer program to provide information about the cancer center and its related programs.
- Collaborate with marketing and communications departments in events as needed, and assist in developing patient education materials.
Requirements
- Graduate from an accredited school of nursing with a BSN required.
- Valid State of Florida Registered Nurse (RN) license and BLS certification required.
- Minimum 2 years of relevant work experience.
