Clinical Outreach Specialist

About the Role:
As a Clinical Outreach Specialist at Open Sky Community Services, you will play a vital role in delivering evidence-based interventions to individuals with mental illness or dual diagnosis. Your primary responsibility will be to act as a liaison between individuals served, community agencies, and families to coordinate treatment and support services.
Key Responsibilities:
- Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
- Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
- Provide side-by-side support and coaching to help individuals socialize and increase their readiness to engage in rehabilitation.
- Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
- Participate in the On-Call rotation.
Requirements:
- High School Diploma, GED or equivalent, required.
- Valid Driver's License, reliable and insured vehicle, and acceptable driving record, required.
- Reliability, willingness to learn, and being open to new opportunities.
