Care Coordinator

SSTAR is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in managing and arranging care activities on behalf of our enrollees, ensuring the appropriate delivery of healthcare services and improving health outcomes.
Key Responsibilities:- Collaborate with treatment teams to develop aftercare plans.
- Act as a liaison between the agency, enrollees, and outpatient providers, community resources, and supports.
- Travel for home-based visits of enrollees.
- Assist clients in assessing appropriate resources, programs, and supports, including completion of necessary paperwork.
- Facilitate education sessions, including health and wellness coaching for individuals and groups.
- Document enrollee information, activities, care plans, and services in an electronic health record.
- Complete Comprehensive Assessments and Care Plans for every engaged member, working with Primary Care Providers on Care Plan approval.
- Facilitate follow-up calls.
- Participate with the total staff to implement program objectives and philosophy.
- Attend meetings and trainings as required.
- High School diploma or equivalent required with at least three years' relevant professional experience; or
- Associate degree and at least one year professional experience in the field; or
- Bachelor's degree in social work, human services, nursing, psychology, sociology, or related field.
- Driver's license required.
- Personal vehicle required (work mileage reimbursement provided).
- Experience with treatment of enrollees with chemical dependence and/or psychiatric impairment.
- Computer skills.
- Strong oral and written skills.
- Ability to function effectively as a treatment team member.
- Evidence of good health, grooming, and professional conduct.
- Possess awareness of own strengths and limitations.
- Bilingual/Spanish a plus.
