Care Coordinator Job Description

SSTAR is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in managing and arranging care activities on behalf of our enrollees, ensuring the appropriate delivery of healthcare services and improving health outcomes.

Key Responsibilities:
  • Collaborate with treatment teams to develop aftercare plans.
  • Act as a liaison between the agency, enrollees, and outpatient providers, community resources, and supports.
  • Travel for home-based visits of enrollees.
  • Assist clients in assessing appropriate resources, programs, and supports, including completion of necessary paperwork.
  • Facilitate education sessions, including health and wellness coaching for individuals and groups.
  • Document enrollee information, activities, care plans, and services in an electronic health record.
  • Complete Comprehensive Assessments and Care Plans for every engaged member, working with Primary Care Providers on Care Plan approval.
  • Facilitate follow-up calls.
  • Participate with the total staff to implement program objectives and philosophy.
  • Attend meetings and trainings as required.
Qualifications:
  • High School diploma or equivalent required with at least three years' relevant professional experience; or
  • Associate degree and at least one year professional experience in the field; or
  • Bachelor's degree in social work, human services, nursing, psychology, sociology, or related field.
  • Driver's license required.
  • Personal vehicle required (work mileage reimbursement provided).
  • Experience with treatment of enrollees with chemical dependence and/or psychiatric impairment.
  • Computer skills.
  • Strong oral and written skills.
  • Ability to function effectively as a treatment team member.
  • Evidence of good health, grooming, and professional conduct.
  • Possess awareness of own strengths and limitations.
  • Bilingual/Spanish a plus.
