Overview:
Southcoast Health System is seeking a skilled RN-LPN-SNF Care Coordinator to join our team. As a key member of our post-acute care team, you will play a vital role in ensuring seamless transitions for patients from acute care to skilled nursing facilities.

Responsibilities:
Under the direction of the VP of Post-Acute Care, you will communicate with skilled nursing facilities to secure bed offers for Southcoast Hospitals. You will also work closely with case managers to facilitate the placement of patients from acute care to skilled nursing facilities. Your duties will include managing the referral to bed offer process, documenting bed offers and bed flows, and creating a strong relationship with skilled nursing facility accounts to secure bed offers.

Qualifications:
Current licensure by the Board of Registration in Nursing (LPN or RN) is required. One to three years of nursing experience is required, with at least two years of experience in an SNF/sub-acute care facility. Proficient computer skills, including experience with various software, database, and electronic records programs, are also required. Must be fully vaccinated against seasonal influenza and COVID-19 virus or exempt for medical or personal reasons.
