Nurse Care Coordinator

About the Role:
The RN Care Coordinator plays a vital part in our multidisciplinary team, working closely with physicians, social workers, and other healthcare professionals to ensure seamless patient care.
Key Responsibilities:
- Collaborate with healthcare teams to develop and implement effective care plans.
- Facilitate patient transitions and discharges, ensuring timely and efficient care delivery.
- Work with social workers to address psychosocial patient care issues and develop complex transition/discharge plans.
- Serve as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information.
- Maintain hospital compliance with Quality Improvement Organization (QIO) series and CMS guidelines.
- Utilize medical necessity criteria to assess appropriateness of care levels and settings.
- Participate in patient safety initiatives and contribute to a culture of safety.
Requirements:
- Licensed Registered Nurse in the State of Virginia or eligible.
- Minimum of three (3) years of nursing experience in an acute care setting.
- Baccalaureate Degree in Nursing from an accredited School of Nursing, or a Bachelor's degree in a biological, health or social sciences field from an accredited school.
Preferred Qualifications:
- Academic healthcare experience.
- One (1) year of UM/Care Coordination experience.
- Clinical experience with specialty patient population.
- Master's Degree in Nursing or a healthcare related field from an accredited program.
Work Environment:
This role requires strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. The RN Care Coordinator will interact with patients, family members, healthcare professionals, community and state agencies, and serve as an advanced clinical resource to the team.
Physical Requirements:
This position requires prolonged sitting, walking, and the ability to lift 20-50 lbs. The incumbent must be able to handle multiple priorities, frequent and intense customer interactions, and adapt to frequent changes.
