Job Description

As a Clinical Care Coordinator, you will be responsible for coordinating patient care and promoting health equity in our underserved communities. This will involve working closely with our healthcare team to assess patient needs, develop individualized care plans, and provide education and support to patients and their families. You will also be responsible for communicating with patients and their families to ensure that their needs are met and that they are receiving the care they need.

Required Skills and Qualifications

To be successful in this role, you will need to have a strong foundation in nursing principles and practices, as well as excellent communication and interpersonal skills. You will also need to be able to work effectively in a team environment and be able to think critically and make sound decisions. A graduate degree in nursing and a current RN license are also required for this role.

Benefits
