LifeLong Medical Care is seeking a dedicated Registered Nurse to join our team at the William Jenkins Center. As a key member of our multidisciplinary team, you will provide exceptional primary care services to patients with complex medical conditions. Your passion for working collaboratively in a team model of care will be essential in delivering high-quality patient care.

Responsibilities:

  • Coordinate team-based care by focusing on the intersection between medical and social needs of patients.
  • Act as a resource for support staff for triaging walk-in and telephonic care.
  • Facilitate chronic disease management activities.
  • Coordinate primary care clinic services with other providers and agencies.
  • Coordinate with Care Transitions team for all hospitalized patients to ensure timely follow-up and return to primary care.
  • Use clinician-approved standing orders to order labs and preventive screenings, and to refill routine medications.

Qualifications:

  • Ability to effectively coordinate care with medical assistants and other support staff.
  • Commitment to providing primary care services for the underserved.
  • Strong organizational, administrative, and problem-solving skills.
  • Ability to effectively present information to others.
  • Ability to work in a team-oriented environment.

Benefits:

  • Competitive salary range: $102,000 - $118,260 per year.
