Client Care Specialist

In this role, you will be instrumental in linking clients and their families with customized services tailored to their specific requirements. Your responsibilities will include managing referrals and coordinating both internal and external resources to ensure comprehensive support for our clients. Key Responsibilities:
- Evaluating client needs through initial intake and screening processes.
- Connecting clients and families with appropriate services and resources.
- Ensuring seamless continuity of care among the client's healthcare team.
- Facilitating collaboration among internal and external parties to create integrated, person-centered treatment plans.
- Assessing and addressing clients' identified Social Determinants of Health (SDOH).
- Collaborating closely with both internal and external referral sources.
- Participating in the rotation of intake weeks for new inquiries and referrals.
- Assisting clients with the annual update of intake documentation and release of information.
- Documenting and maintaining precise and timely summaries of care coordination.
- Supporting assigned providers with client scheduling and record requests for care coordination.
- Tracking and monitoring referrals effectively.
- Attending multidisciplinary team meetings and accurately documenting notes and attendance.
- Monitoring the Encounter Alert Service (EAS) and following up with clients within 24 hours post-discharge from emergency or hospital care.
- Collecting and entering necessary data into the client's Electronic Care Record (ECR).
- Overseeing providers assigned to caseloads.
- Performing additional duties as assigned by the supervisor.
To qualify for the Care Coordinator position, candidates must meet the following criteria:
Minimum Qualifications:
- High School Diploma or GED.
- Must demonstrate a non-judgmental attitude towards diverse lifestyles, races, and economic statuses.
- Ability to pass a background study and comply with relevant regulations.
- Must be at least 21 years of age and possess a valid driver's license.
- Knowledge and experience in behavioral health programming.
- Completion of two years of college in a healthcare or human services field, or three years of relevant experience, preferably in a behavioral health clinic or social services setting.
- Exceptional client-centered care, interpersonal, and organizational skills.
- Capacity to manage multiple tasks in a fast-paced environment.
- Strong oral and written communication abilities.
- Team-oriented mindset.
- Sound judgment in emotionally charged and demanding situations.
- Familiarity with community resources.
- Adaptability to challenging situations and changes.
- Proficiency in using Electronic Medical Record (EMR) systems.
- Compassionate, empathetic, and understanding approach towards clients and their families.
- Ability to collaborate professionally with internal and external providers and agencies.
- Commitment to maintaining confidentiality.
- Non-judgmental perspective regarding diverse lifestyles, races, and economic statuses.
- Ability to sit for extended periods while working at a desk and using a computer.
- Capability to lift up to 15 pounds occasionally.
