Job Title: Clinical Nurse Coordinator

We are seeking a highly skilled Clinical Nurse Coordinator to join our team at Montefiore Medical Center. As a Clinical Nurse Coordinator, you will play a critical role in the coordination and delivery of patient care, working closely with our interdisciplinary team to ensure the highest level of care for our patients.

Responsibilities:
  • Systematically assess the health care needs of patients and families, and develop plans to address these needs.
  • Provide direct care to assigned patients, including administering medications, performing procedures, and providing education to patients and families.
  • Collaborate with the healthcare team to develop and implement care plans, and to ensure that patients receive comprehensive and coordinated care.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure that all needs are met.
Qualifications:
  • New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an RN required.
  • Certification preferred in specialty or efforts to achieve certification.
  • BSN degree required and 5 years clinical experience, two of which must be in the specialty or related service; or combination of education and experience acceptable to Clinical Director of Nursing or designee.
