Job Summary:The Clinical Nurse Coordinator will be responsible for:
- Coordinating patient care services, including developing and implementing individualized care plans
- Collaborating with healthcare teams to ensure seamless care transitions and optimal patient outcomes
- Providing education and support to patients and families to promote health and wellness
- Maintaining accurate and up-to-date patient records and reports
- Participating in performance improvement initiatives to enhance patient care and safety
To be successful in this role, you will need:
- A New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an R.N.
- Certification preferred in specialty or efforts to achieve certification
- BSN degree required and 5 years clinical experience, two of which must be in the specialty or related service
- Demonstrated clinical leadership, problem-solving skills, competency, and principles of Montefiore Excellence
