Job Title: Elder Services Coordinator

We are seeking a highly skilled and compassionate Elder Services Coordinator to join our team at the Puyallup Tribe of Indians. As a key member of our Elder Services department, you will play a vital role in providing exceptional care and support to our elderly community.

Job Summary:

The Elder Services Coordinator will be responsible for planning, organizing, and directing the day-to-day operations of our Elder Services department. This includes assessing, coordinating, and advocating for the needs of our elderly clients, as well as providing nursing care and support as approved by the Executive Director of Elder Services.

Key Responsibilities:
  • Assist with reviewing and updating policies and procedures related to community health needs
  • Conduct intakes, develop care plans, and make referrals for Tribal Elders who require financial or other social service assistance
  • Perform case-finding, health teaching, health counseling, and coordination of care with primary health providers and other professionals
  • Implement and conduct orientation and in-service training for department staff and participate in guidance and educational programs
  • Participate in committees and task forces to improve elder care within the community
  • Compile statistical data and write narrative reports summarizing quality management findings and outcomes
  • Provide casework and group work services to elders and vulnerable adults in the community
  • Work closely with families and designated POA/guardians to coordinate clinical services for the elder
  • Identify and resolve barriers to promote interdisciplinary collaboration for elder care needs
  • Ensure the care team and process of care is sustainable long-term and provide a caring relationship to elders and families
  • Coordinate meetings for timely clinical decision-making and optimal handovers across the continuum of care
  • Develop and promote a staff philosophy that encourages elders to do things for themselves and retain feelings of independence and self-esteem
  • Develop strategies to achieve department goals and objectives
  • Participate in the hiring of departmental personnel and complete performance evaluations
  • Train staff on the importance of trustworthiness and confidentiality
  • Make decisions, exhibit sound judgment, and support reasoning for decisions
  • Provide general nursing care and coordination in the home using a biopsychosocial approach
  • Assess elders' health conditions and needs, prepare nursing care plans, and educate elders and families on self-care and well-being concepts
  • Conduct home, hospital, or other healthcare community visits regularly
  • Follow instructions, respond promptly to management direction, and ensure accurate and timely documentation of visits/encounters
  • Use approved BBP, infection prevention/control measures, and proper disposal techniques for sharps, equipment, and infectious waste
  • Maintain strict confidentiality, adhere to the Privacy Act, and HIPPA Laws
Requirements:
  • Bachelor's Degree in Nursing and current Registered Nurse license in Washington State
  • Minimum of two years previous experience working with an elder population coordinating social services, including experience addressing abuse in the Elder and/or vulnerable adult population
  • Effective supervisory experience
  • Intermediate to advanced computer skills in word processing, spreadsheets, and databases
  • Sufficient supervisory experience to demonstrate the ability to assess current staff strengths and weaknesses and make recommendations for development and training

The noise level is usually moderate. The physical exertion is low to moderate. Work is performed in an office setting and in the field at Elder's homes. There are frequent employee contacts and interruptions during the day. May require use of personal protective equipment for nursing care. Travel to Puyallup Tribal Elders homes throughout the service area. May interact with upset individuals.
