Key Responsibilities- Client Advocacy: Collaborate with the Clinical Manager to act as an internal case manager and advocate for clients in the Habilitation program.
- Person-Centered Planning: Participate in Person-Centered Planning processes to ensure client care needs are met.
- Client Care Coordination: Collaborate with clients, families, Case Managers, Hab Techs, and other teams to implement and evaluate client care needs and individual goals identified in the Person Centered/Individual Support Plan.
- Client Progress Monitoring: Make regular client visits to evaluate client progress and monitor that client services are implemented according to the Person Centered/Individual Support Plan and the service definitions.
- Goal Evaluation: Evaluate the progress of goals identified in the Person Centered/Individual Support Plan.
- Communication: Maintain ongoing communication with Case Managers/Care Coordinators.
- Supervision: Carry out supervision of Habilitation Technicians according to policy and procedure and as mandated by state regulation.
- Client Orientations: Participate in client specific orientations as directed by the Clinical Manager.
- Bachelor's Degree: A bachelor's degree is required for this role.
- Qualified Professional: Must be a Qualified Professional with a minimum of 2 years of experience with I/DD population post-human services bachelor's degree, or 4 years of experience with I/DD population post-bachelor's degree not in human services field.
- Supervisory Experience: Prior supervisory experience is a plus.
- Leadership Skills: Demonstrated record of successfully taking on increased responsibility.
- Communication Skills: Excellent communication and interpersonal skills.
