Delaware Human Services Case Manager Role

Key Responsibilities:
• Conduct thorough assessments of client needs and develop comprehensive service plans to address their health and human service requirements.
• Work closely with clients, family members, and community resources to provide support and guidance in navigating available services.
• Collaborate with healthcare providers, social services agencies, and other stakeholders to ensure seamless delivery of services and achieve optimal outcomes.
• Maintain accurate and up-to-date records of client interactions, service plans, and progress.
• Develop and implement effective case management strategies to ensure clients receive timely and appropriate services.
• Provide education and support to clients on available services, benefits, and resources to promote self-sufficiency.
• Participate in multidisciplinary teams to develop and implement behavioral health services and treatment plans.
• Monitor and evaluate the effectiveness of service plans and make adjustments as needed to ensure optimal outcomes.
• Develop and maintain relationships with community partners to ensure access to services and resources for clients.
• Stay current with changes in laws, regulations, and policies affecting health and human services to ensure compliance and optimal service delivery.
• Analyze data to identify trends and areas for improvement in service delivery and develop strategies to address these gaps.
We offer a competitive salary of $43,000 - $63,000 per year, depending on experience, and a comprehensive benefits package that includes medical, dental, and vision coverage, as well as life insurance and disability benefits.
This is a casual/seasonal position, and the starting salary is $43,000 per year, with the potential for advancement based on performance.
Applicants must have a bachelor's degree in a related field and six months of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and build strong relationships with clients, family members, and community partners.
