You will escalate barriers to the plan of care through appropriate channels and work with physicians to consider alternate levels of care if patients are not meeting clinical stability. You will also collaborate with social work to manage patients with high acuity psychosocial needs.

Responsibilities:
Collaborate with physicians and interdisciplinary teams to implement plans of care and treatment for patients with advanced heart failure.
Review medical records to anticipate clinical stability and have a thorough understanding of the patient prior to follow-up.
Ensure patients are progressing through clinical milestones and adjust treatment plans as needed.

Escalate barriers to the plan of care through appropriate channels.
Collaborate with social work to manage patients with high acuity psychosocial needs.

Qualifications:
Current RN license from the State of Florida.
Minimum of 2 years nursing experience in acute care or ambulatory setting.
Current certification in Cardiac/Heart Failure.
