Job Title: Therapist Case Manager

Job Summary:

We are seeking a skilled Therapist Case Manager to join our team at Healthcare for the Homeless Inc. The successful candidate will provide direct mental health therapy and case management services to adults experiencing homelessness with various disabling conditions and psychosocial stressors.

Key Responsibilities:

  • Provide behavioral health assessments and diagnosis, individual and group therapy, and intensive case management services to support clients' overall health and housing stability.
  • Collaborate with other providers in an integrated, interdisciplinary care team to coordinate care and attend interdisciplinary meetings.
  • Develop and implement integrated treatment plans using SMART goals, and ensure relevant authorizations are completed according to established guidelines.
  • Act as a liaison between clients and landlords/property managers, and provide representative payee services to select clients.
  • Document client information in a timely and accurate manner, and maintain compliance with HIPAA and other relevant health care-related IT security regulations.

Requirements:

  • Master's Degree from an approved School of Social Work
  • Current license as an LMSW or LCSW-C
  • Personal vehicle and valid Maryland driver's license required
  • One year experience providing therapy to individuals with substance use and mental health disorders preferred
