Job Summary
Transformation Healthcare is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in helping individuals with chronic illness, mental health, and substance use disorders access rehabilitation programs and community services.
Key Responsibilities
- Meet with clients and their families to assess their needs and develop personalized treatment plans.
- Connect clients with professional services, including therapy, medical care, and social support.
- Maintain consistent communication with clients to monitor their progress and make adjustments to their treatment plans as needed.
- Attend team meetings and participate in interdisciplinary approaches to ensure comprehensive care.
- Admit new clients, review records and applications, and conduct orientations.
- Determine clients' requirements by completing intake interviews and reviewing therapist evaluations.
- Establish treatment programs, set schedules and routines, and coordinate services.
- Monitor cases, verify clients' attendance, and observe and evaluate treatments and responses.
- Maintain clients' records, review case notes, and log events and progress.
- Communicate clients' progress through weekly interdisciplinary meetings and evaluations.
- Prepare clients for discharge, review and amplify discharge plans, and coordinate post-discharge requirements.
Requirements
- High School Diploma or GED
- 3+ years of customer service experience, preferably in human services
- Demonstrated commitment to customer service
- Valid in-state driver's license and a driving record with no more than 2 points within the last 3 years
- Ability to sit for extended periods, stand and walk frequently, and lift light objects up to 25 pounds
Language
English (required), Spanish (preferred)
Salary
$18.00 - $23.00/hourly
