Job Summary

Transformation Healthcare is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in helping individuals with chronic illness, mental health, and substance use disorders access rehabilitation programs and community services.

Key Responsibilities

  • Meet with clients and their families to assess their needs and develop personalized treatment plans.
  • Connect clients with professional services, including therapy, medical care, and social support.
  • Maintain consistent communication with clients to monitor their progress and make adjustments to their treatment plans as needed.
  • Attend team meetings and participate in interdisciplinary approaches to ensure comprehensive care.
  • Admit new clients, review records and applications, and conduct orientations.
  • Determine clients' requirements by completing intake interviews and reviewing therapist evaluations.
  • Establish treatment programs, set schedules and routines, and coordinate services.
  • Monitor cases, verify clients' attendance, and observe and evaluate treatments and responses.
  • Maintain clients' records, review case notes, and log events and progress.
  • Communicate clients' progress through weekly interdisciplinary meetings and evaluations.
  • Prepare clients for discharge, review and amplify discharge plans, and coordinate post-discharge requirements.

Requirements
  • High School Diploma or GED
  • 3+ years of customer service experience, preferably in human services
  • Demonstrated commitment to customer service
  • Valid in-state driver's license and a driving record with no more than 2 points within the last 3 years
  • Ability to sit for extended periods, stand and walk frequently, and lift light objects up to 25 pounds

Salary
$18.00 - $23.00/hourly
