About this Role

We are seeking a skilled Case Manager to join our team at Johns Hopkins Bayview Medical Center. As a Case Manager, you will play a critical role in coordinating patient-centered care across the continuum, ensuring seamless transitions and optimal progression in care.

Key Responsibilities
  • Collaborate with patients, families, social workers, physicians, and the interdisciplinary team to provide leadership and advocacy in care coordination.
  • Manage clinical resources and transition planning for patients within an assigned caseload, identifying and eliminating barriers to care delivery.
  • Work closely with the care team to ensure timely and effective transitions, promoting patient safety and satisfaction.
  • Develop and implement individualized care plans, incorporating patient goals and preferences.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.

Requirements
  • Completion of an accredited RN Nursing Program; BSN preferred.
  • One year of RN experience required; preferred experience in case management, discharge planning, or utilization review.
  • Valid Maryland Nursing License required.
