Case Manager

Tuerk House, a leading non-profit behavioral health organization, is seeking a skilled Case Manager to join our team at our Main Campus in Baltimore.
Job SummaryThe Case Manager will play a crucial role in providing comprehensive support and guidance to individuals navigating their recovery journey. This position will collaborate with a multidisciplinary team to assess clients' needs and facilitate stabilization to optimize treatment outcomes.
Key Responsibilities- Collaborate with clinical staff to identify clients for case management services
- Conduct evaluations to determine clients' needs related to vocational, educational, or housing support
- Assist clients in applying for necessary public services and benefits
- Create and maintain a list of community resources and agencies to provide essential services to clients
- Develop comprehensive, individualized care plans addressing medical care, treatment readiness, medications, housing, financial assistance, legal issues, and substance abuse/mental health treatment
- Work with medical and support staff to gather assessment information and medical records, facilitating discharge planning and continuity of care
- Coordinate services required to implement the care plan effectively
- Make referrals to address clients' social service and support needs
- Monitor clients to assess the efficacy of the service care plan and adjust as necessary
- Assist clients in developing long-term solutions to financial emergencies, linking them to appropriate resources
- Participate in required meetings and training sessions
- Collect and submit required data for monthly reports
- Bachelor's degree in Social Work or related field
- Extensive knowledge of psychiatric illnesses and substance abuse disorders, including treatment options and psychopharmacology
- Familiarity with community resources, networking techniques, and emergency stabilization protocols
- Proficient in computer skills, including email communication and documentation
- Excellent communication skills with active listening abilities
- Expertise in documenting medical records accurately and comprehensively
- Strong organizational and time-management abilities, with attention to detail
- Demonstrated responsibility, professionalism, and adherence to HIPAA and state compliance regulations
