Mental Health Therapist and Case Manager

Healthcare for the Homeless Inc is seeking a skilled Mental Health Therapist and Case Manager to join our team. As a Mental Health Therapist and Case Manager, you will provide direct mental health therapy and case management services to adults experiencing homelessness with various disabling conditions and psychosocial stressors.
Key Responsibilities:- Provide behavioral health assessments and diagnosis, individual and group therapy, and intensive case management services to support clients' overall health and housing stability.
- Collaborate with other providers in an integrated, interdisciplinary care team to coordinate care and engage in problem-solving.
- Develop and implement integrated treatment plans using SMART goals, and ensure relevant authorizations are completed according to established guidelines.
- Act as a liaison between clients and landlords/property managers, and provide representative payee services to select clients.
- Master's Degree from an approved School of Social Work.
- Current license as an LMSW or LCSW-C.
- Personal vehicle and valid Maryland driver's license required.
