Job Title: Service Coordinator

Silvercrest Senior Residences is a vital resource for low-income seniors, providing affordable housing and supportive services. As a Service Coordinator, you will play a critical role in ensuring the well-being and independence of our residents.

Job Summary:
  • Identify the human service needs of residents and develop service plans to address these needs.
  • Collaborate with community agencies to provide services and resources to residents.
  • Educate residents and staff on issues related to aging in place and service provision.
  • Assist the Manager in promoting resident independence and quality of life.
Responsibilities:

As a Service Coordinator, you will be responsible for:

  • Conducting needs assessments and developing service plans for residents.
  • Identifying and connecting residents with community resources and services.
  • Providing education and support to residents and staff on issues related to aging in place.
  • Collaborating with the Manager to promote resident independence and quality of life.
Requirements:

To be successful in this role, you will need:

  • A strong understanding of aging in place and human services.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.

Silvercrest Senior Residences offers a competitive salary and benefits package, including:

  • Full-time employment (40 hours/week).
  • Salary range: $24.00-$29.00/hour.
