Home Health Care Registered Nurse Position

Montefiore Medical Center is seeking a skilled Registered Nurse (RN) to join our Home Health team in Bronx, New York.
Job SummaryWe are looking for a compassionate and experienced RN to provide high-quality patient care in the comfort of patients' homes. As a Home Health RN, you will work closely with our interdisciplinary team to develop and implement individualized care plans that promote patient independence and well-being.
Responsibilities- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
- Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans to address these needs.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
- Document patient progress and outcomes in accordance with regulatory requirements.
- 2 years of experience as a Home Health RN.
- 1 year of experience in a supervisory or leadership role.
- Ability to educate and motivate staff.
- Good communication skills (written, verbal, oral).
- Knowledge of Medicare conditions of participation.
- BSN (Masters preferred).
- NYS RN valid license.
- NYS RN registration.
- Valid driver's license and ability to provide own transportation during working hours for the purpose of making home visits.
