**Job Summary:**

Amazing Home Care is seeking a highly skilled and experienced Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our team, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Responsibilities:

  • Admit and reassess patients in accordance with physician orders
  • Coordinate and supervise Home Health Aides in the delivery and discharge of patient care
  • Initiate home care visits and reassess patient needs and progress on a regular basis
  • Develop and implement individualized plans of care under the guidance of physicians
  • Monitor and document patient progress, identifying areas for improvement and implementing changes as needed
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and caregivers on disease management, medication adherence, and individualized treatment plans

Requirements:

  • Active New York State RN License
  • COVID-19 vaccination
  • At least 1 year of experience in a hospital and home care setting
  • Excellent communication and time-management skills
  • Ability to travel to multiple patients per day
  • Bilingual in English and Spanish/Creole/Cantonese/Mandarin/Russian (a plus)

Preferred Qualifications:

  • UAS experience
