Home Health Care Registered Nurse Position

Montefiore Medical Center is seeking a skilled Registered Nurse (RN) to join our Home Health team in Bronx, New York.
Job SummaryWe are looking for a compassionate and experienced RN to provide high-quality patient care in the comfort of patients' homes. As a Home Health RN, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide direct patient care, including assessments, medication management, and wound care.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare team members.
- Conduct regular home visits to assess patient progress and adjust care plans as needed.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document patient information accurately and thoroughly in accordance with regulatory requirements.
- 2 years of experience in Home Health care with a focus on CHHA (Certified Home Health Aide) management.
- 1 year or more of supervisory experience in a healthcare setting.
- Ability to educate and motivate staff to provide high-quality patient care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
- Knowledge of Medicare conditions of participation and regulatory requirements.
- BSN (Bachelor of Science in Nursing) degree, with a Master's degree preferred.
- NYS RN valid license and registration.
- Valid driver's license and ability to provide own transportation during working hours for home visits.
