Job SummaryWe are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health RN, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with our interdisciplinary team to ensure seamless care transitions.
Responsibilities- Provide comprehensive nursing care to patients in their homes, including wound care, medication management, and education on disease management and self-care.
- Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans to address these needs.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and effective communication.
- Document patient progress and outcomes in accordance with regulatory requirements and Montefiore Medical Center's policies and procedures.
- Participate in ongoing education and training to stay current on best practices and evidence-based care.
- Current RN licensure in New York State.
- 2 years of experience in home health care or a related field.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Knowledge of Medicare conditions of participation and regulatory requirements.
