Home Health Registered Nurse Opportunity

Montefiore Medical Center is seeking a skilled Registered Nurse to join our Home Health team in Bronx, New York.

Job Summary

We are looking for a compassionate and experienced Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Registered Nurse, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure seamless care transitions.

Responsibilities
  • Provide comprehensive patient assessments and develop personalized care plans
  • Deliver high-quality patient care in accordance with established standards and protocols
  • Collaborate with healthcare teams to ensure effective care coordination and communication
  • Supervise and educate Certified Home Health Aides (CHHAs) to ensure optimal patient outcomes
  • Stay up-to-date with industry developments and best practices in home health care
Requirements
  • Current New York State RN registration
  • 2 years of experience in home health care or a related field
  • Ability to motivate and educate staff
  • Knowledge of Medicare conditions of participation
