Position Overview:

The role involves delivering specialized pharmaceutical care services in an outpatient setting, collaborating with healthcare professionals to manage patient medication therapies, educate patients, and oversee the preparation and dispensing of medications.

The focus is on promoting high-quality, evidence-based, and cost-effective pharmaceutical care within designated clinical areas.

Key Responsibilities:
Adhere to organizational policies, principles of responsibility, and relevant laws.
Conduct thorough medication history assessments upon referrals from healthcare providers.

Develop comprehensive treatment plans based on diagnoses, which may encompass pharmacotherapy and lifestyle adjustments, ensuring continuity of care.

Identify, address, and avert potential medication-related issues, including:

untreated conditions, inappropriate drug choices, suboptimal dosages, overdose, medication non-adherence, adverse drug reactions, drug interactions, and unnecessary medications.

Initiate, modify, monitor, and recommend drug therapy adjustments under established protocols, utilizing information gathered from patient interactions, clinical monitoring, and medical records.

Coordinate and analyze laboratory tests related to drug therapy, including serum drug levels and other relevant laboratory assessments.

Monitor patients to evaluate and enhance outcomes, optimize drug use, ensure adherence, and minimize adverse drug reactions.

Provide patient education regarding disease states, including expected effects, side effects, adherence importance, and specific instructions.

Ensure continuous improvement in the pharmaceutical care services offered.
Deliver educational sessions to healthcare providers and staff through various formats, including presentations and newsletters.
Conduct drug utilization reviews, comprehensive medication assessments, and medication reconciliations.
Utilize pharmacy information systems and other relevant technologies.

Accurately document patient information in medical records and comply with policies and regulatory standards. Foster a collaborative work environment.

Perform additional duties as necessary.

Qualifications:

Minimum of 1200 hours of experience in the past five years working under collaborative drug therapy management protocols, including the initiation, adjustment, discontinuation, and refilling of medications for patients with chronic conditions, or successful completion of an ASHP Accredited PGY-1 residency.

A qualified candidate should possess at least ONE of the following credentials or experiences:

Board Certified Oncology Pharmacist (BCOP) OR completion of a PGY2 in oncology practice OR 1000 hours of experience in sterile compounding oncology pharmacy with direct clinical protocol management, with the condition of achieving BCOP within three years of employment OR successful completion of an ASHP accredited PGY1 Residency (within three years), including an oncology pharmacy rotation, with the condition of achieving BCOP within three years of employment. Education: Degree in Pharmacy meeting the minimum educational requirements for licensure.

License, Certification, Registration: Pharmacist License (California) required at hire. National Provider Identifier required at hire. Basic Life Support certification within three months of hire.

Knowledge Required:


In-depth understanding of pharmacological principles, medical terminology, disease management, age-appropriate therapies, and other relevant information for effective pharmaceutical care management.

Knowledge of federal and state regulations governing pharmacy services and the pharmacist's scope of practice.
Demonstrated experience in aseptic techniques and preparation of sterile IV products and hazardous materials, as necessary.
Familiarity with current drug information sources and medical literature.

Skills and Abilities Required:
Proven ability to extract relevant information from patients and other sources for effective medical management.
Demonstrated teaching and evaluation skills.
Strong verbal and written communication skills.
Analytical and problem-solving capabilities.
Experience collaborating within a multidisciplinary patient care team.

Preferred Qualifications:
Doctorate of Pharmacy (Pharm.D.).
Completion of a PGY-2 clinical pharmacy specialty residency or fellowship. Board certification in a relevant practice area. Advanced Practice Pharmacist License.
