Position Overview:

The Clinical Pharmacy Specialist in Ambulatory Care is responsible for delivering comprehensive pharmaceutical care services in collaboration with healthcare professionals. This role includes managing patient medication regimens, providing educational support to patients, and overseeing the preparation and dispensing of medications in accordance with established protocols.

The specialist is dedicated to promoting high-quality, evidence-based, and cost-effective pharmaceutical care within assigned clinical areas.

Key Responsibilities:
Adheres to organizational policies, procedures, and applicable laws.
Conducts thorough medication history assessments based on referrals from healthcare providers.

Develops individualized treatment plans that may encompass pharmacotherapy and lifestyle adjustments, ensuring continuity of care.

Identifies, addresses, and prevents medication-related issues, including:

untreated conditions, inappropriate medication choices, suboptimal dosages, overdoses, missed doses, adverse drug reactions, drug interactions, and unnecessary medications.

Initiates, modifies, monitors, and recommends adjustments to drug therapy under established protocols, utilizing information gathered from patient interactions, objective monitoring data, and comprehensive reviews of medical records.

Coordinates and interprets laboratory tests related to drug therapy, including serum drug concentrations and other relevant laboratory evaluations.

Monitors patients to assess and enhance therapeutic outcomes, medication use, and adherence, while identifying and mitigating adverse drug reactions and unnecessary medication use.

Provides education to patients regarding their health conditions, including expected effects, side effects, the importance of adherence, and any specific instructions.

Ensures continuous improvement in the quality of pharmaceutical care services delivered.
Educates healthcare providers and staff through presentations, newsletters, and direct interactions.
Conducts medication utilization reviews, comprehensive medication assessments, and medication reconciliation.
Utilizes pharmacy information systems and related technology.

Accurately documents relevant information in patient medical records and other systems in compliance with policies and regulatory standards. Fosters a collaborative environment that supports labor-management partnerships.

Performs additional duties as assigned.

Qualifications:

Minimum of 1200 hours of experience within the last five years in collaborative drug therapy management, including the initiation, adjustment, discontinuation, and refilling of medications for patients with chronic conditions, or successful completion of an ASHP Accredited PGY-1 residency.

A qualified candidate should possess at least ONE of the following credentials or experiences:

Board Certified Oncology Pharmacist (BCOP) OR completion of a PGY2 residency in oncology OR a candidate with 1000 hours of experience in sterile compounding oncology pharmacy with direct clinical protocol management, with the condition of obtaining BCOP certification within three years of employment OR a candidate who has completed an ASHP accredited PGY1 Residency (within three years) with an oncology pharmacy rotation during the residency year and the condition of achieving BCOP certification within three years of employment. Education: Degree in Pharmacy meeting the minimum educational requirements for licensure.

License, Certification, Registration: Pharmacist License (California) required at hire. National Provider Identifier required at hire. Basic Life Support certification within three months of hire.

Knowledge and Skills Required:

In-depth knowledge of pharmacological principles, medical terminology, disease management, and therapeutic approaches relevant to pharmaceutical care.

Familiarity with federal and state regulations governing pharmacy services and the pharmacist's scope of practice.
Demonstrated experience in aseptic techniques and preparation of sterile IV products and hazardous materials, as necessary.
Awareness of current drug information sources and medical literature.

Preferred Qualifications:
Doctorate of Pharmacy (Pharm.D.)
Completion of a PGY-2 clinical pharmacy specialty residency or fellowship.
Board certification in a relevant practice area.
Advanced Practice Pharmacist License.
