Clinical Pharmacy Specialist in Ambulatory Care

Position Overview:
The Ambulatory Care Pharmacist delivers comprehensive pharmaceutical care services in collaboration with healthcare professionals. This role involves managing patient medication regimens, providing educational support to patients, and overseeing the preparation and dispensing of medications according to established protocols.
The pharmacist is dedicated to promoting high-quality, evidence-based, and cost-effective pharmaceutical care within clinical settings.Key Responsibilities:
Adhere to organizational policies, procedures, and relevant laws.
Conduct thorough medication history assessments based on referrals from medical staff.
Develop and implement treatment plans that may encompass pharmacotherapy and lifestyle changes, ensuring continuity of care.
Identify, address, and prevent medication-related issues, including:untreated conditions, inappropriate medication choices, inadequate dosages, overdoses, missed doses, adverse drug reactions, drug interactions, and unnecessary medications.
Within established protocols, initiate, modify, monitor, and recommend adjustments to drug therapies based on comprehensive patient evaluations, including interviews, clinical parameters, and medical records.
Oversee and interpret laboratory tests related to medication therapy, including serum drug levels and other relevant tests.Monitor patients to assess and enhance outcomes, medication usage, and adherence, while minimizing adverse drug reactions and preventing unnecessary medication use.
Educate patients about their health conditions, covering expected effects, side effects, adherence importance, and specific instructions.
Ensure continuous improvement in the pharmaceutical care services provided.Deliver educational sessions to healthcare providers and staff through various formats, including presentations and newsletters.
Conduct drug utilization reviews, comprehensive medication assessments, and medication reconciliations.
Utilize pharmacy information systems and related technologies effectively.
Maintain accurate documentation in patient medical records and comply with regulatory standards.
Support a collaborative work environment focused on partnership and teamwork.Perform additional duties as needed.
Qualifications:
Minimum of 1200 hours of experience in the past five years under collaborative drug therapy management protocols, or successful completion of an ASHP Accredited PGY-1 residency.
Candidates should possess at least one of the following credentials or experiences:Board Certified Oncology Pharmacist (BCOP) OR completion of a PGY2 in oncology OR 1000 hours of experience in sterile compounding oncology pharmacy with direct clinical protocol management, with a commitment to achieving BCOP within three years of employment OR completion of an ASHP accredited PGY1 Residency (within three years) with an oncology pharmacy rotation and a commitment to achieving BCOP within three years of employment.
Education: Degree in Pharmacy meeting the minimum educational requirements for licensure.Licensure: Pharmacist License (California) required upon hire.
National Provider Identifier required upon hire.
Basic Life Support certification within three months of hire.
Knowledge and Skills:
Extensive knowledge of pharmacological principles, medical terminology, disease management, and therapeutic approaches relevant to patient care.
Understanding of federal and state regulations governing pharmacy services and pharmacist practice.Experience in aseptic techniques and preparation of sterile IV products and hazardous materials, as required.
Familiarity with current drug information sources and medical literature.
Preferred Qualifications:
Doctorate of Pharmacy (Pharm.D.)
Completion of a PGY-2 clinical pharmacy specialty residency or fellowship.
Board certification in a relevant practice area.
Advanced Practice Pharmacist License.
