Job Summary

MedStar Health is seeking a highly skilled and experienced RN Clinical Mentor to join our team. As a Clinical Mentor, you will play a critical role in guiding and supporting newly hired and current clinicians in their professional development and growth.

Key Responsibilities
  • Contribute to the achievement of established goals and objectives, adhering to department policies, procedures, quality standards, and safety standards.
  • Serve as a mentor and guide for field trainers/preceptors, providing new hires with the learning experiences they need to meet clinical competency requirements.
  • Collaborate with the clinical training specialist and Operations Director to support an individualized clinical orientation program for new staff, updating the program as needed to meet learning needs.
  • Provide feedback to new and current associates, conducting peer-to-peer reviews and being available as a clinical resource when needed.
  • Serve as a field educator, providing feedback and suggestions to implement best practices and standardize procedures.
  • Maintain a caseload of patients, meeting the expectation of care described in the corresponding clinical job description, and perform admissions, revisits, recerts, discharge, and joint visits to ensure quality patient care delivery and excellence in patient experience.
  • Collaborate with the Operations Director and RN clinical training specialists in building a caseload for new staff and facilitating their transition from orientee to functional member of the team.
  • Act as a field mentor to clinical staff, providing the Operations Director and/or RN clinical training specialist with input regarding team members' work performance, recommending and providing additional education based on clinical skill needs or problems identified through the QA process or at the request of the staff member or Operations Director and/or RN clinical training specialist.
  • Conduct chart reviews during newly hired associates' first year, ensuring that admissions, recerts, and discharges in the patient documentation are accurate and that proper utilization and clinical needs are being achieved.
  • Perform Joint Visits and participate in mock surveys as directed.
  • Assist with service recovery, communicating with patients, caregivers, and interdisciplinary staff to resolve actual or potential issues with the patient.
  • Maintain effective working relationships with other departments, participating in multidisciplinary quality and service improvement teams, meetings, and on committees representing the department and agency in community outreach efforts.
  • Enhance growth and development and enrich personal knowledge and skill through participation in educational programs and affiliations.
  • Responsible for clinical outcomes, patient experience, and utilization management.
  • Provide direct patient care and case management for complex care patients, as well as supervision of LPNs and HHAs.
  • Demonstrate advanced knowledge base of Oasis documentation/accuracy.
  • Ensure compliance of documentation regulatory requirements, including care plan building, goal writing, and guiding clinicians and promoting proficiency in documentation.
Requirements
  • Bachelor's degree in Nursing from an accredited program or combination of education with 3 years relevant nursing experience, including 3 years experience in a certified home care agency or hospice.
  • 3-4 years relevant nursing experience with Associate's degree or 2 years relevant nursing experience with Bachelor's degree.
  • 3-4 years experience in a certified home care or hospice program.
  • Prior preceptor experience preferred.
  • Valid RN license in all jurisdictions in which the employee provides services to patients, Maryland, DC, or Virginia, or any combination based on work location(s).
  • Valid State Driver's License in good standing with a dependable vehicle for transportation.
  • CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days.
  • Additional unit/specialty certifications may vary by department or business unit.

  • Demonstrated ability to teach, direct, and guide professionals to gain desired results.
  • Demonstrated ability to function at a high level of independence and autonomy.
  • Strong leadership and delegation skills required.
  • The ideal candidate will have a passion for teaching and excellent interpersonal skills, including verbal and written communication.
  • IV therapy, Foley catheter, venipuncture, wound care/vac, and comprehensive medical-surgical skills.
  • Organizational skills.
  • Excellent verbal and written communication skills.
  • Basic computer skills required.
  • Knowledge of home equipment/DME, home care reimbursement, and regulations governing LPNs and HHAs.
