Outpatient Outreach Coordinator

Job Summary:
The Washington University in St. Louis is seeking a highly motivated and compassionate individual to fill the role of Outpatient Outreach Coordinator. As a key member of our team, you will serve as a point of contact for individuals eligible for the Life Outside of Violence (LOV) program, providing outreach and information about the program to potential participants in hospital and community health clinic settings.
Primary Responsibilities:
- Provide support to patients and families following onset of injury and after discharge from Level 1 Trauma Centers.
- Respond to incoming patients and/or their families that have been violently injured or meet any other LOV criteria when they present to affiliated outpatient clinics for follow-up care appointments.
- Check in with patients that meet LOV criteria and are currently utilizing or seeking post-injury care from qualifying hospital clinic or community health clinic.
- Correspond with Outpatient H.E.A.L. Team worker to ensure proper communication and continuity of care.
- Check in with clinic social worker to ensure all patients that are eligible for LOV programming have been identified and provided with crisis support and engagement for possible program enrollment.
Requirements:
- Bachelor's degree or combination of education and/or experience may substitute for minimum education.
- No specific certification is required for this position.
- No specific work experience is required for this position.
Preferred Qualifications:
- Five years of relevant experience in effectively fostering strong relationships with people in neighborhoods that are affected by violence and trauma.
- Experience working with at-risk, gang-involved, or youth and/or adults impacted by violence.
- Personal experience in overcoming violence/incarceration/violence-related injuries/gang involvement/the criminal justice system.
