Key Responsibilities:
- Provide high-quality patient care in a home or facility setting, focusing on dignity, compassion, and empathy.
- Assist patients with their personal needs, including bathing, dressing, and grooming.
- Work collaboratively with the healthcare team to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in ongoing education and training to enhance your skills and knowledge in patient care.
Requirements:
- High school diploma or equivalent required.
- Certified State Tested Nursing Assistant (STNA) certification in the State of Ohio.
- One year of experience in patient care preferred.
- Certification in Hospice & Palliative Medicine (CHPNA) preferred, required within two years of employment.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
