Key Responsibilities:

  • Provide high-quality patient care in a home or facility setting, focusing on dignity, compassion, and empathy.
  • Assist patients with their personal needs, including bathing, dressing, and grooming.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to enhance your skills and knowledge in patient care.

Requirements:

  • High school diploma or equivalent required.
  • Certified State Tested Nursing Assistant (STNA) certification in the State of Ohio.
  • One year of experience in patient care preferred.
  • Certification in Hospice & Palliative Medicine (CHPNA) preferred, required within two years of employment.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
