Hospice Care Specialist

At Affinity Care of Ohio, we are seeking a compassionate and dedicated Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will play a vital role in providing high-quality care to patients and their families in their time of need.
Key Responsibilities:- Provide personal care and support to patients, including bathing, dressing, and grooming.
- Assist patients with daily living activities, such as feeding, toileting, and ambulation.
- Offer emotional support and companionship to patients and their families.
- Document patient care and progress in electronic medical records.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Provide education and support to patients and their families on end-of-life care and hospice services.
- Completion of a state-approved certified nursing assistant course or equivalent training.
- Current Healthcare Provider CPR certification.
- Covid vaccination.
- High school diploma or General Equivalency Degree (GED).
- Ability to read and follow written instructions and document care given.
- Understanding of hospice philosophy and ability to provide specialized care to the terminally ill.
- Prior experience in hospice, orderly, acute, rehabilitative, or long-term care facility.
- Basic computer skills to document in electronic medical records (EMR).
- At least 18 years of age.
- Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
