Job Title: Hospice Care Specialist

At Affinity Care of Ohio, we are seeking a compassionate and dedicated Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will play a vital role in providing high-quality care to patients with terminal illnesses and their families.

Key Responsibilities:
  • Provide personal care and support to patients, including bathing, dressing, and grooming.
  • Assist patients with daily living activities, such as feeding, toileting, and ambulation.
  • Monitor patients' physical and emotional well-being, reporting any changes or concerns to the RN Case Manager.
  • Provide emotional support and companionship to patients and their families.
  • Assist with light housekeeping and laundry tasks to maintain a clean and comfortable living environment.
  • Document patient care and progress in electronic medical records (EMR).
Requirements:
  • Completion of a state-approved certified nursing assistant course or equivalent training.
  • Current Healthcare Provider CPR certification.
  • High school diploma or General Equivalency Degree (GED).
  • Ability to read and follow written instructions and document care given.
  • Understanding of hospice philosophy and ability to provide specialized care to the terminally ill.
  • Basic computer skills to document in EMR.
  • At least 18 years of age.
  • Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
