Overview:
The Clinical Pharmacy Specialist is entrusted with ensuring the delivery of safe, effective, and timely medication therapy. In this pivotal role, Clinical Pharmacy Specialists engage in direct patient-centered care within a decentralized practice environment.

In this clinical capacity, Clinical Pharmacy Specialists are involved in all essential components of the medication-use system, providing comprehensive and tailored pharmaceutical care to patients in their designated areas.

Pharmaceutical care services encompass a range of activities, including evaluating patient needs, integrating age and disease-specific factors into drug therapy and patient education, modifying care based on patient responses, and executing clinical interventions to identify, mitigate, and avert medication-related adverse events.

Clinical Pharmacy Specialists act as departmental resources and points of contact for other departments or external entities.

They also engage in clinical research and practice enhancement initiatives, along with patient care quality and regulatory compliance efforts aimed at refining medication-use processes or pharmacy practice.

Clinical Pharmacy Specialists provide education and training related to medication and practice, actively serving as mentors for Doctor of Pharmacy students and pharmacy residents.

These specialists will contribute to the quality management program with the objective of enhancing services by monitoring processes, analyzing data, implementing improvement strategies, and assessing the effectiveness of those strategies.


Clinical Pharmacy Specialists will oversee the clinical management of patients undergoing various specialty therapies for medical conditions, including but not limited to Hepatitis C, inflammatory disorders (gastroenterology, dermatology, asthma, and allergy), pulmonary diseases (idiopathic pulmonary fibrosis, cystic fibrosis), neurological conditions (multiple sclerosis, migraine headache), dyslipidemia, osteoporosis, and oncology.

Leads designated patient service lines, clinical areas, and therapeutic programs.


Facilitates pharmaceutical care services, direct patient care initiatives, and medication utilization systems within assigned services and care areas to ensure drug utilization activities align with patient care needs, evidence-based best practices, and regulatory standards.


Proactively identifies practice-related challenges that necessitate evaluation and facilitates clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice.

Ensures pharmacy is an integral component of the healthcare delivery system and promotes the enhancement and expansion of pharmacy services/programs.

Delivers direct patient care.

Maintains proficiency in the clinical pharmacy program.

Collaborates as an active member of the multidisciplinary team with healthcare providers to deliver patient-centered care.

Identifies high-risk patients and implements interventions to enhance quality and safety.

Makes appropriate evidence-based, patient-centered medication recommendations.

Oversees the workflow of liaisons to ensure timely review and processing of orders.

Manages the prior authorization and appeals process.

Provides clinical support to technician staff as necessary.


Conducts and documents initial and follow-up assessments and consultations in accordance with program policies and procedures and URAC and ACHC accreditation requirements.

Participates in a rotating after-hours on-call process.

Delivers pharmaceutical services throughout the health system.

Maintains proficiency in hospital computer systems and medication ordering systems.

Ensures accurate, safe, timely, and appropriate medication therapy based on patient age and needs.


Completes critical patient monitoring and reviews patient profiles/charts to identify, prevent, or mitigate drug-related issues, improper drug or dose selection, sub-therapeutic dosage, overdosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.

Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.).

Actively participates in stewardship activities and restricted medication programs.

Engages in pharmacy operations and medication dispensing as assigned.

Maintains proficiency in and actively participates in operational programs and specialty pharmacy areas as appropriate for job assignment.

Facilitates experiential education and practice advancement.

Maintains proficiency in preceptor roles and actively demonstrates education/mentorship skills.

Develops training experiences/competencies for students, residents, and staff, creating new opportunities for teaching and training.

Identifies, designs, and implements enhancements in the medication-use system to promote patient safety, optimize therapeutic outcomes, or control costs.

Qualifications:
Doctor of Pharmacy degree from an ACPE accredited school of Pharmacy is required.
PGY1 residency or 5 years of clinical practice (community, ambulatory care, PBM, or hospital) required.
PGY2 residency preferred.
Specialty pharmacy experience preferred.
Experience with EPIC suite products preferred (EMR, Compass Rose, Willow Ambulatory).

Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in areas appropriate for clinical assignments required.

Licensed pharmacist in the state of practice.
Pharmacy preceptor license within 90 days of start date.
Must achieve American Society of Health-System Pharmacists (ASHP) Residency Preceptor status within 18 months of hire.
Must obtain Certified Specialty Pharmacist (CSP) certification or other applicable board (e.g. BCOP, BCACP, BCPS, BCMTMS, BCGP) certification within 18 months of hire.
