Position OverviewRole Summary:

The Clinical Pharmacy Specialist is entrusted with ensuring the safe, effective, and timely administration of medication therapy. In this pivotal role, Clinical Pharmacy Specialists deliver expert patient-centered care within a decentralized healthcare environment. They engage in all critical components of the medication-use process while offering comprehensive and tailored pharmaceutical care to patients in designated areas. Their services encompass assessing patient requirements, integrating age and disease-specific factors into medication therapy and patient education, modifying care based on patient feedback, and executing clinical interventions to identify, mitigate, and avert adverse medication events.

Clinical Pharmacy Specialists act as key resources and liaisons to various departments and external organizations. They also spearhead clinical research and practice enhancement initiatives, alongside patient care quality and regulatory compliance projects aimed at refining medication-use processes and pharmacy operations.

Key Responsibilities:

  • Oversees designated patient service lines, clinical domains, and therapeutic programs.
  • Facilitates pharmaceutical care services, direct patient care initiatives, and medication utilization systems within assigned areas to ensure alignment with patient care needs, evidence-based practices, and regulatory standards.
  • Proactively identifies practice-related challenges that necessitate evaluation and leads clinical research projects, quality improvement efforts, or educational initiatives for healthcare providers to advance practice.
  • Ensures that pharmacy is an integral component of the healthcare delivery framework and promotes the enhancement and expansion of pharmacy services and programs.
  • Delivers direct patient care and maintains expertise in the clinical pharmacy program.
  • Collaborates as an active member of the multidisciplinary team to provide patient-centered care.
  • Identifies high-risk patients and implements strategies to enhance quality and safety.
  • Provides evidence-based, patient-focused medication recommendations.
  • Supervises the workflow of liaisons to guarantee timely review and processing of medication orders.
  • Manages the prior authorization and appeals processes.
  • Offers clinical support to pharmacy technicians as necessary.
  • Conducts and documents initial and follow-up assessments and consultations in accordance with program policies and accreditation requirements.
  • Participates in a rotating on-call schedule for after-hours support.
  • Maintains proficiency in hospital information systems and medication ordering platforms.
  • Ensures accurate, safe, timely, and appropriate medication therapy tailored to patient age and needs.
  • Completes critical patient monitoring and reviews patient profiles to identify, prevent, or mitigate drug-related issues.
  • Communicates effectively with healthcare providers and caregivers.
  • Actively engages in stewardship activities and restricted medication programs.
  • Participates in pharmacy operations and medication dispensing as assigned.
  • Maintains proficiency in operational programs and specialty pharmacy areas relevant to the role.
  • Facilitates experiential education and practice advancement by mentoring students, residents, and staff.
  • Identifies, designs, and implements enhancements in the medication-use system to improve patient safety, optimize therapeutic outcomes, or manage costs.

Qualifications:

  • Doctor of Pharmacy degree from an accredited institution is required.
  • Completion of a PGY1 residency or a minimum of 5 years of clinical practice experience is required.
  • PGY2 residency is preferred.
  • Experience in specialty pharmacy is preferred.
  • Familiarity with EPIC suite products is preferred.
  • Extensive knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and evidence-based medicine in relevant areas is required.
  • Must hold a valid pharmacist license in the state of practice.
  • Pharmacy preceptor license must be obtained within 90 days of employment.
  • Must achieve American Society of Health-System Pharmacists (ASHP) Residency Preceptor status within 18 months of hire.
  • Must obtain Certified Specialty Pharmacist (CSP) certification or other relevant board certification within 18 months of hire.
