Key Responsibilities:
This role involves a combination of telephonic patient engagement and provider collaboration, both virtually and in-person, alongside market representatives.
Develop comprehensive market strategies aimed at motivating healthcare providers and leadership of large or complex groups to enhance Stars measures, targeting a rating of 4 STARS or higher in pharmacy-related metrics such as Statin Use in Patients with Diabetes, Statin Use in Patients with Cardiovascular Disease, and Medication Adherence for Diabetes, Cholesterol, and Hypertension.
Provide detailed analytical insights into Stars and HEDIS reporting, including executive summaries for plan and provider groups that highlight best practices and address patient-related challenges.

Act as the primary expert on all STARS-related initiatives within designated markets, collaborating within a matrix structure that includes Quality corporate operations and Regional/Market operations.

Lead or engage in presentations during weekly, monthly, bi-monthly, quarterly, and/or annual business review meetings concerning STAR activities, summarizing provider and large group performance as required by Quality or Regional leadership.

Analyze and assess the structure and characteristics of provider groups and large organizations, as well as their operational dynamics, to identify effective strategies and approaches.
Assist in the creation of training and analytical resources for Stars and HEDIS.
Identify key decision-makers and other essential personnel within provider groups, focusing on recognizing barriers to achieving desired outcomes and tailoring communications and efforts accordingly.
Develop user-friendly, solution-oriented initiatives to support the success of practices.
Conduct outbound calls to members to gather information regarding HEDIS and Quality measures related to their care, as well as to identify any potential gaps in care.
Perform annual medication reviews of both prescription and non-prescription medications, emphasizing patient assessment and counseling, ensuring safe and effective medication use, and identifying potential drug interactions.
Conduct pharmacotherapy assessments based on patients' claims, medical history, and disease states.
Evaluate patients for medication adherence challenges and devise plans to enhance adherence to prescribed medications.
Deliver patient-centered, clinically collaborative medication consultations.
Create a follow-up list of action items for patients and/or prescribers post-consultation.
Maintain an extensive knowledge base of chronic disease states and advancements in pharmacy practice for adult and geriatric populations, serving as a reliable source of information on medication utilization in therapy.
Perform all other related duties as assigned.

Qualifications:
Doctor of Pharmacy degree or equivalent experience.
Must possess an active, unrestricted pharmacy license in Texas.
Minimum of 5 years of clinical pharmacy practice experience.
At least 2 years of experience with Medicare STARs and HEDIS measures.
Minimum of 2 years of experience in provider-facing roles, including provider education.
At least 2 years of experience in data analysis and/or quality chart reviews, with the ability to review data and provide actionable recommendations for improvement.
1+ years of experience in a call center environment.
Proficient in Microsoft Word, Excel, Outlook, and PowerPoint.
Ability to thrive in a fast-paced and highly matrixed environment.
Located or within commuting distance of the market being supported.
