Position Overview:


The Healthcare Social Services Coordinator plays a vital role in assisting the Social Services department to ensure that all medically related emotional and social requirements of patients are fulfilled in accordance with relevant laws, regulations, and Life Care standards.


Qualifications and Experience:

  • High school diploma or equivalent
  • Previous experience in healthcare preferred

Key Responsibilities:

  • Make autonomous decisions when necessary
  • Familiarity with practices and procedures as well as the laws, regulations, and guidelines governing operations in the post-acute care environment
  • Implement and interpret the programs, objectives, policies, and procedures of the department
  • Demonstrate proficiency in all competency areas including but not limited to: patient rights, safety, and sanitation
  • Maintain professional relationships with all colleagues, vendors, etc.
  • Safeguard the confidentiality of all proprietary and/or sensitive information
  • Understand and adhere to company policies including harassment and compliance protocols
  • Exhibit integrity and professionalism by following Life Care's Code of Conduct and completing mandatory compliance training

Core Duties:

  • Support the social services program to address the medically related social and emotional needs of patients while adhering to Federal, State, corporate, and division guidelines
  • Document actions appropriately and promptly, in compliance with State law
  • Assist social worker(s) in providing support to patients and families through education, financial planning assistance, and coordination with community agencies
  • Display exceptional customer service and a positive demeanor towards patients
  • Aid in the safe evacuation of patients
  • Show reliable and consistent attendance
  • Utilize reasoning skills and sound judgment
  • Communicate effectively and collaborate productively within an interdisciplinary team
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during working hours
  • Read, write, speak, and comprehend the English language
