Job Title: Counselor / Case Manager

Job Summary:

We are seeking a skilled and compassionate Counselor / Case Manager to join our team at Evergreen Recovery. As a key member of our residential program, you will provide comprehensive case management and recovery-focused services to residents, assisting them in achieving their goals and overcoming challenges.

Responsibilities:

  • Conduct thorough psychosocial assessments to identify resident needs and develop individualized treatment plans.
  • Collaborate with the clinical team to engage residents, communicate effectively, and coordinate care.
  • Provide group and individual sessions, documenting resident progress and participation.
  • Maintain accurate and timely clinical records, adhering to regulatory requirements.
  • Assist with discharge planning, providing referrals and support as needed.
  • Transport and accompany residents off-site as required.
  • Participate in staffing, in-service trainings, and meetings as scheduled.

Requirements:

  • Current California Driver's License.
  • Valid and current CADC/CATC/SUDCC certification.
  • Ability to monitor and maintain accurate documentation.
  • TB screen, Health Screening, CPR/First Aid certificate within 7 days of hire.
  • Registration or certification up to date at all times to provide substance abuse recovery services.

  • Knowledge of regulatory and accreditation standards, federal, state, and local codes.
  • Qualifications and provision of care in accordance with national acceptable standards of practices.
  • Identifies and utilizes community resources effectively.
  • Manages time effectively to meet competing demands and timelines.
  • Participates in supervision and/or staffing with the clinical coordinator.
  • Solves problems collaboratively with colleagues, consumers, and community partners.
  • Participates in trainings to update knowledge and skills.
  • Effectively uses computer-based systems for data entry, planning, and documenting care and treatment.
  • Communicates effectively using oral and written skills with staff, consumers, family members, community members, and partners.
  • Understanding and working knowledge of recovery principles.
  • Self-determination and independence.

We maintain an at-will policy of employment, which means that employment and compensation may be terminated with or without cause and with or without notice at any time at the option of either the company or the employee.
