Enrich Lives as a Social Services Specialist

At Springbrook Center - Westbrook, ME, we are seeking a dedicated and compassionate Social Services Specialist to join our team. As a key member of our interdisciplinary team, you will work closely with patients, residents, and their families to identify strengths, social, emotional, and mental health needs. Your goal will be to provide patients with the highest practical level of physical, mental, and psychosocial well-being and quality of life.

Key Responsibilities:
  • Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
  • Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
  • Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
  • Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
  • Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Qualifications:
  • Bachelor's degree in social work or human services field required
  • Must possess any certifications/licensures as required by State of employment to practice in long-term care
