Social Services Coordinator

We are seeking a highly skilled and compassionate Social Services Assistant to join our team at Sedgewood Center in Falmouth, ME. As a key member of our social services team, you will provide administrative support to our patients and residents, ensuring a seamless transition and adjustment throughout their stay.
Key Responsibilities- Collaborate with patients and their families to provide support and resource information
- Respond to issues identified by patients and their families
- Facilitate patient transfer within the nursing center to ensure a smooth transition
- Participate in the healthcare decision-making process under the direction of a qualified social worker or consultant
