About the Role

We are seeking a highly skilled and compassionate Social Services Assistant to join our team at Sedgewood Center - Falmouth, ME. As a key member of our social services team, you will provide administrative support to patients and their families, ensuring a seamless transition and adjustment throughout their stay.

Key Responsibilities
  • Collaborate with patients and their families to provide support and resource information.
  • Respond to issues identified by patients and their families, providing timely and effective solutions.
  • Facilitate patient transfer within the nursing center, ensuring a smooth transition and adjustment.
  • Participate in the healthcare decision-making process, working under the direction of a qualified social worker and/or consultant.

Requirements
